Radiesse is Superior to Hyaluronic Acid for Wrinkles!

As reported in the February 2008 issue of Dermatologic Surgery, a study involving 60 patients compared the effects of calcium hydroxylapatite (Radiesse) versus stabilized hyaluronic acid (Juvederm, Restylane) for the treatement of nasolabial folds. The study was randomized and split-face, that is, one side received the hylauronic acid and the other side the radiesse. Patients were re-evaluated and 6, 9, and 12 months by an observer that did not know which side had received which treatment. The results were strongly in favor of Radiesse. At 12 months, 79% of treated folds were still improved vs. only 43% of hyaluronic acid folds. Both evaluators and patients preferred the Radiesse side two to one over the hyaluronic acid side.

Another study in the February 2007 issue of the Journal of Academic Dermatology also reported long-lasting correction of nasolabial folds with Radiesse. Among patients that had previously received hyaluronic acid injections, such as Restylane, Radiesse was preferred due to long-lasting effects and more cosmetically pleasing correction. Radiesse was well-tolerated and had no significant adverse effects in the study.

Of note, hyaluronic acids, such as Juvederm, are still the preferred injection material for lip augmentation. Calcium hydroxylapatite is know to sometimes cause small nodules under the skin that could be more easily visible under thin lip skin. This is not a significant issue when injecting in the thicker skin of the rest of the face.

Injectable Fillers: Juvederm(tm) and Restylane(tm)

Restylane(tm) and now Juvederm(tm) are cosmetic surgical materials that have recently become household names. In this era of direct-to-consumer pharmaceutical company advertising and promotion, this is not so surprising. The current preoccupation with anti-aging formulas and cosmetic surgery has propelled the popularity of these two bio-chemicals. Riding on the heels of BOTOX(™) (botulinum toxin) for cosmetic use, Restylane(tm) has allowed doctors and other healthcare practitioners to administer significantly rejuvenating treatments for an affordable price and “over lunch.”

What Restylane(tm) and Juvederm(tm) actually are is not as commonly known as what they do. They are composed of hyaluronic acid. This is a substance that is part of the extracellular matrix of the skin. It is part amino-acid and part sugar and acts as a filling/cushioning material between cells. An interesting feature of it is that it binds to water molecules and swells to a much larger volume that the actual material itself occupies. Thus it forms a gel. Naturally occurring hyaluronic acid, if injected into your skin, is quickly broken down and goes away. Restylane(tm), Juvederm(tm), and other injectable hyalans differ in that they are “cross-linked” to increase their stability. This means that a gel of some hardness is formed that can last for months at a time.

A subtle difference between Juvederm(tm) and Restylane(tm) is that the former is composed of different size fragments of cross-linked hyaluronic acid while Restylane(tm) is said to be composed of more uniformly sized fragments. The makers of Juvederm(tm) contend that this gives their product increased smoothness of flow despite increased cross-linking, which would otherwise make their gel harder.

Both substances are injected mainly into the lips and nasolabial folds (the smile lines between the corner of the mouth and the nose). Lips can be increased in fullness, pouting, or get increased definition of the edges. Facial depressions that cause shadow-lines, such as the nasolabial folds are also good targets for these fillers. Other target areas included deep lines in the corners of the mouth and deep frown furrows.

Generally, these fillers are injected using a fine needle and syringe. Discomfort is minimized by using topical anesthetic, but dental anesthetic injections may be necessary to adequately numb the lips, which are very sensitive.

Good results can usually be achieved with 1-2 syringes. Both Restylane(tm) and Juvederm(tm) last approximately 6 months, although the makers of Juvederm(tm) contend that it lasts 2-3 months longer. In my experience, the fillers do not last as long in the lips as in the rest of the face. I believe that this is due to increased use of the lips with talking and eating.

Juvederm is a dermal filler similar to Restylane. That can be used for smoothing of deep facial grooves, such as the smile line between the corner of the mouth and the nose, and to give you plumper lips. Juvederm is made by Allergan and is said to be smoother in application and last somewhat longer than Restylane.

Move Over, Botox: Wrinkle ‘Filler’ Is New Option

Nov. 17, 2006 — Nearly 8.5 million minor cosmetic procedures were performed in the United States last year, including microdermabrasion and botox, which are designed to get rid of wrinkles. But now there’s a new player in town — Juvederm.

Juvederm was approved by the FDA last summer and will be available in your doctor’s office this January. Some say the new treatment might just win the race for eternal youth.

Debbie Lane, a 50-year-old personal trainer and mother of two, is happy with her looks. But as she’s gotten older, she’s noticed a definite change.

“For most of my life, I’ve been very involved in physical activity,” she said. “And it helps me mentally and physically, but it helps me from the shoulders down. It really can’t do too much above the shoulders.

“I feel there is a deep fold from the bottom of my nose down to the chin,” she added. “And that’s the area I hope Dr. [Frederic] Brandt can help me with.”

Brandt, a cosmetic dermatologist, thought he could.

“As we age, certain wrinkles in our face are caused by volume loss — the sunken cheeks, the lines that run from our nose to our mouth the shrinking lips,” Brandt said. “So fillers like Juvederm restore volume in the face.”

The procedure is simple: A numbing cream prepares you for the needles, and then a few well-placed injections of Juvederm fills out the wrinkles.

In the right hands, Juvederm can get some amazing results — but as with unlicensed Botox parties, there are risks. Someone can get in some real health trouble if they don’t go to a trained medical professional.

“There are risks to any procedure, such as bruising, swelling and infection, and these risks are minimized when you go to a professional — preferably a cosmetic dermatologist or a plastic surgeon,” said Dr. Doris Day, author of “Forget the Facelift.”
